The island may have been inhabited intermittently for almost 1,800 years. King James IV used the island to train his hawks and also reportedly sent children to Inchkeith to be raised by a mute woman, in an attempt to determine if language was learned or innate. During the 15th and 16th centuries, victims of the plague and syphilis from Edinburgh were sent to Inchkeith for quarantine. In 1547, during the Rough Wooing, English soldiers occupied Inchkeith and began building a fort as a base for attacks against Scotland.
